1040-SR (Web-based) | How to Add Custom Consent Forms
💡to add one of the consent forms, you can go to CONSENT FORMS from the configuration menu. But if you want to add a custom consent form, follow the steps below
From the Main Menu go to Configuration > Consent Forms
Select +Add Consent Form
Name the Consent Form and provide a description
Display Disclosure Limitations allows you to determine if any limitations may be specified
Required for E-file allows you to require the consent before e-filing
Select the + next to Consent Form Terms
Name the Term and either select Input Only or specify the Definition for the term
When you are finished, click the checkmark next to the term to save
Save the Consent Form
